Licensed Practical Nurse - Home Care Team Case Assistant (Full Time, Days)

Summary

Utilizes clinical experience to coordinate patient services under the direction of the team manager/designee. Coordinates daily patient schedules, requests additional services, and ensures timely service referrals.

Responsibilities

• Prioritizes and coordinates daily patient schedules
• Requests additional services such as aides, therapy, social work, interpreter services
• Validates supplemental orders and processes per policy
• Remains available to assist in other duties as needed
• Manages multiple phone calls in a timely and professional manner
• Provides field staff assistance and resources for day-to-day issues
• Checks email in a timely manner to provide follow-up
• Collects, organizes, and reviews various programs for information and reporting
• Updates the EMR per agency standards
• Performs functions related to transfers or discharges as required by regulation
• Builds and maintains strong, communicative relationships with the team
• Provides exemplary customer service
• Performs general office duties/projects including answering phones, documentation/order processing, copying, filing, mailing, and obtaining/ordering office supplies

Requirements

• Graduate of an accredited school of licensed practical nursing
• A valid NYS license as a Licensed Practical Nurse
• One year of experience in home care setting or medical office preferred
• Excellent interpersonal skills and customer service skills
• Ability to multi-task in a fast-paced environment
• Proficient use of Microsoft office preferred

Qualifications

• Education: LICENSES / CERTIFICATIONS - BLS - Basic Life Support - American Heart Association (AHA), RN - Registered Nurse - New York State Education Department (NYSED)

Physical Requirements

• M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.

Pay

The listed base pay range is a good faith representation of current potential base pay for a successful full-time applicant. It may be modified in the future and eligible for additional pay components. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
