Position Summary
Captures patient services under the direction of the team manager/designee. Coordinates daily patient schedules, requests additional services, and ensures timely assignment of referrals.
Key Responsibilities
- Manages multiple phone calls in a timely and professional manner, providing appropriate follow-up to ensure caller needs are met.
- Provides field staff assistance and resources for day-to-day issues under the direction of the team manager/designee.
- Checks email in a timely manner, providing follow-up to ensure sender's needs are attended to appropriately.
- Collects, organizes, and reviews various programs for information and reporting, initiating follow-up as needed.
- Updates the EMR according to agency standards.
- Performs functions related to transfers or discharges as required by regulation.
- Builds and maintains strong, communicative relationships with the team.
- Provides exemplary customer service.
- Performs general office duties/projects, including answering phones, documentation/order processing, copying, filing, mailing, and obtaining/ordering office supplies.
- Affords assistance in orientation of new staff, role modeling, and participation in quality assurance activities.
- Attends team meetings and other internal meetings as requested.
- Performs other duties as assigned.

Minimum Qualifications
- One year of experience in home care setting or medical office preferred.
- No required licenses/certifications.
