Home Health Registered Nurse Case Manager - Full Time

About the role

Provide personalized, one-on-one nursing care in patients' homes, promoting recovery, safety, and independence. Conduct comprehensive assessments, develop patient-centered care plans, and deliver skilled interventions such as wound care, infusions, medication management, and post-operative support. Educate and empower patients and caregivers to manage symptoms and medications confidently. Monitor progress, identify early changes, and communicate with the care team to adjust plans and prevent hospitalizations. Coordinate care across providers, ensuring continuity and high-quality outcomes. Complete accurate documentation to support compliance and quality metrics.

Qualifications

• Active RN license in the practice state
• Minimum of one year of clinical nursing experience
• Patient-focused, organized, and motivated to deliver high-quality care
• Reliable transportation, valid driver's license, and auto insurance
• CPR certification or ability to obtain
