Home Health Registered Nurse Case Manager Full Time

About the Role

We are seeking a dedicated Registered Nurse (RN) for full-time home health care within the Raleigh and Durham areas. This role emphasizes compassionate, patient-centered care, clinical excellence, and teamwork to improve health outcomes in the home setting.

Responsibilities

• Provide individualized, in-home nursing care to promote recovery, safety, and independence.
• Perform comprehensive assessments and develop patient-centered care plans.
• Administer skilled interventions such as wound care, infusions, medication management, and post-operative care.
• Educate patients and caregivers to manage symptoms and treatments confidently.
• Monitor patient progress, identify changes, and collaborate with healthcare teams to adjust care plans.
• Ensure care continuity and complete accurate documentation to support quality outcomes.

Requirements

• Active RN license in the practice state.
• At least one year of clinical nursing experience.
• Patient-focused, organized, and motivated to deliver high-quality care.
• Valid driver’s license, reliable transportation, and CPR certification (or ability to obtain).
