Team Case Assistant - Home Care (Full time, day shift)

Position Summary

Captures patient services under the direction of the team manager/designee. Coordinates daily patient schedules, requests additional services, and ensures timely assignment of referrals.

Key Responsibilities

• Manages multiple phone calls in a timely and professional manner, providing follow-up to ensure caller needs are met.
• Provides field staff assistance and resources for day-to-day issues under the direction of the team manager/designee.
• Checks email in a timely manner, providing follow-up to ensure sender’s needs are addressed appropriately.
• Collects, organizes, and reviews various programs for information and reporting, initiating follow-up as necessary.
• Updates the EMR according to agency standards.
• Performs functions related to transfers or discharges as required by regulation.
• Builds and maintains strong, communicative relationships with the team.
• Provides exemplary customer service.
• Performs general office duties/projects, including answering phones, documentation/order processing, copying, filing, mailing, and obtaining/ordering office supplies.
• Affords assistance in orientation of new staff, role modeling, and participation in quality assurance activities.
• Attends team meetings and other internal meetings as requested.
• For ElderONE only: Coordinates participant services and staff scheduling, creates and schedules case assignments, and assists with timekeeping as needed.
• Provides supportive services to Home Health Aide operations of ElderONE, coordinates transitions for participants through care, and assures appropriate routing of calls.

Minimum Qualifications

• No formal education or certifications required.

Desired Attributes (for ElderONE only)

• One year of experience working with a frail or elderly population preferred.

Physical Requirements

Sedentary Work - Exerting up to 10 pounds of force occasionally. Jobs are sedentary if walking and standing are required only occasionally and all other sedentary criteria are met.

Pay Range

$17.75 - $23.03
