About the role

Licensed Occupational Therapist (OT) – Home Health HTS is currently seeking a dedicated and compassionate Occupational Therapist to join our team in providing exceptional care to home health patients in Columbus, Indiana, and the surrounding areas. In this position, the Occupational Therapist will provide therapy services to residents in their individual homes or onsite at our partner senior living facility. Responsibilities

• Assess patients' functional abilities and develop personalized treatment plans to improve independence and quality of life.
• Monitor and document patient progress, adjusting treatment plans as necessary.
• Educate patients and their families on therapeutic exercises and strategies to support ongoing health and wellness.
• Collaborate with healthcare teams, caregivers, and facility staff to ensure comprehensive patient care.
• Maintain accurate and timely documentation in compliance with regulatory standards.

Requirements

• Applicants must have an active Indiana Occupational Therapist license or must have the necessary qualifications to earn an Indiana license.
• Applicants must be legally eligible to work in the United States.
• Proof of eligibility will be required upon offer of employment.

Qualifications

• Earning an advanced degree from a Commission on Accreditation in Occupational Therapy Education‑accredited occupational therapist education program.
• Passing the state licensure exam.
• Fulfilling the state’s requirements for licensure.
