Occupational Therapist - Home Health

About the role

We are seeking a dedicated Occupational Therapist (OT) for a home health role that emphasizes patient-centered care.

Responsibilities

• Evaluate patients’ functional abilities, mobility, and safety at home
• Develop personalized treatment plans to enhance independence and quality of life
• Provide therapeutic interventions, including retraining in daily activities and fabrication of adaptive devices
• Educate patients, families, and caregivers on safety and ongoing care
• Assess home environments for safety risks and recommend modifications
• Support patients returning to work through functional evaluations
• Collaborate with interdisciplinary teams and participate in discharge planning
• Document care accurately within electronic medical records
• Supervise Certified Occupational Therapy Assistants and oversee home health aide care when applicable

Requirements

• Graduate of an accredited OT program
• Current NBCOT registration and state licensure
• Valid driver’s license and reliable transportation

Skills

• Excellent clinical judgment and communication skills
• Ability to build trust with patients and families

Preferred Qualifications

• Minimum one year of clinical experience, home health experience a plus
• CPR certification
