Occupational Therapist - Home Health

About the role

Provide occupational therapy to patients unable to perform daily tasks due to illness or injury. Collaborate with a home health team including RNs, LPNs, CNAs, HHAs, PTs, and SLPs to develop and implement personalized care plans.

Responsibilities

• Assess patients, identify deficits, set therapy goals, and document progress.
• Assist with exercises to enhance fine motor skills and coordination.
• Recommend adaptive equipment to improve safety and independence, such as grab bars and shower chairs.
• Evaluate fall risks and suggest strategies for home safety.
• Educate patients and families on care plans, exercises, and self-care practices.

Requirements

• Graduate of an accredited Occupational Therapy Program with an active OT license in Virginia.
• Minimum of 2 years of occupational therapy experience, preferably in home healthcare.
• CPR certification and knowledge of home health regulations.
