About the role
We are seeking a compassionate and organized IRIS Consultant to support adults, seniors, and individuals with disabilities through Wisconsin's IRIS program.
Responsibilities
- Build trusted relationships with participants
- Develop and update individualized support plans
- Conduct monthly contacts and quarterly in-person visits
- Promote participant choice, independence, and community involvement
- Monitor health, safety, and program outcomes
- Educate participants on program eligibility and resources
- Collaborate with community organizations, healthcare providers, and resource centers
- Maintain accurate documentation and complete compliance requirements
- Assist with functional assessments
- Work independently within a support team
Qualifications
- Bachelor’s degree in Social Work, Human Services, or related fields with at least one year of experience working with vulnerable populations
- OR four or more years of direct experience serving older adults or individuals with disabilities
- OR current IRIS Consultant in good standing with a Wisconsin DHS-approved exemption
