About the role
Support adults, older adults, and individuals with disabilities through case management, care coordination, and community-based support within Wisconsin's IRIS program.
Responsibilities
- Develop and maintain participant relationships
- Assist in creating and updating individual support plans
- Conduct regular contacts and visits
- Promote self-determination and independence
- Monitor health and safety
- Educate participants on program resources
- Collaborate with community partners
- Maintain accurate documentation
- Support functional assessments
Qualifications
- Bachelor’s degree in a relevant field with at least one year of experience working with disabled or elderly populations, or equivalent direct experience
