IRIS Consultant - Hybrid, Full Benefits, PSLF Eligible

About the role

We are seeking a compassionate and organized IRIS Consultant to support adults, older adults, and individuals with disabilities through Wisconsin's IRIS program.

Responsibilities

• Build trusted relationships with participants
• Develop and update individualized IRIS Support and Services Plans
• Conduct monthly contacts and quarterly in-person visits
• Promote participant choice, independence, and community involvement
• Monitor health, safety, and program outcomes
• Educate participants on IRIS enrollment, eligibility, and resources
• Collaborate with community organizations, healthcare providers, and resource centers
• Maintain accurate case documentation and compliance
• Assist with long-term care assessments
• Work independently within a support team

Qualifications

• Bachelor’s degree in Social Work, Human Services, Psychology, or related field with at least one year of experience working with individuals with disabilities or older adults
• Four or more years of relevant experience
• Current IRIS Consultant status in good standing

Preferred Skills and Experience

• Case management, care coordination, disability services, long-term care, social work, community-based services, Medicaid waiver programs, behavioral health, and person-centered planning
• Strong communication, relationship-building, and cultural awareness skills
• Proficiency with technology and ability to work independently
• Bilingual abilities in Spanish, Hmong, Arabic, Russian, ASL, or other Wisconsin languages are encouraged

Additional Requirements

• Valid driver’s license and reliable transportation
• Ability to travel within the service area and meet insurance standards
