IRIS Consultant - Hybrid, Full Benefits, PSLF Eligible
About the role
We are seeking a compassionate and organized IRIS Consultant to support adults, seniors, and individuals with disabilities through Wisconsin's IRIS Program. This role involves building trusted relationships with participants, assisting in developing and updating individualized support plans, conducting monthly contacts and quarterly in-person visits, promoting self-determination, independence, and community involvement, monitoring health, safety, and program outcomes, educating participants on program requirements and resources, collaborating with community organizations, healthcare providers, and resource centers, maintaining accurate case documentation and compliance, supporting annual functional screens when needed, and working independently within an interdisciplinary team.
Qualifications
- Bachelor’s degree in Social Work, Human Services, or related field with at least one year of relevant experience, or four or more years of direct service experience
- Experience in case management, care coordination, disability services, or related areas is preferred
- Strong communication, relationship-building, and cultural awareness skills
- Proficiency with technology and a valid driver’s license with reliable transportation
- Fluency in additional languages such as Spanish, Hmong, or ASL is highly valued
