About the role
We are seeking a compassionate and organized IRIS Consultant to support adults, older adults, and individuals with disabilities in Wisconsin's IRIS (Include, Respect, I Self-Direct) Program. This role involves case management, care coordination, social work, and community-based support to help participants maintain independence, achieve personal goals, and exercise self-direction.
Responsibilities
- Build trusted relationships with participants
- Develop and update individualized support plans
- Conduct regular contacts and visits
- Promote participant choice and self-direction
- Monitor health and safety
- Educate clients about program requirements
- Collaborate with community organizations
- Maintain accurate documentation
- Support annual assessments
Qualifications
- A bachelor's degree in related fields with at least one year of experience working with individuals with disabilities or older adults, or
- Four or more years of direct service experience
Experience in case management, care coordination, or disability services is preferred. Candidates should possess strong communication skills, cultural awareness, and proficiency with technology. Bilingual candidates, especially in Spanish, Hmong, Arabic, Russian, or ASL, are encouraged to apply.
