About the role
The IRIS Consultant supports adults, older adults, and individuals with disabilities within Wisconsin's IRIS Program. This full-time, hybrid role involves case management, care coordination, social work, and community support to promote independence, self-determination, and safety.
Responsibilities
- Develop personalized support plans for participants
- Build trusted relationships with participants and families
- Conduct regular contacts and visits to assess needs and progress
- Collaborate with healthcare providers and community organizations
- Maintain accurate documentation and complete required reports
- Educate participants about available resources and services
- Assist with assessments such as Long-Term Care Screens
Qualifications
- A bachelor's degree in social services or a related field with at least one year of relevant experience
- OR four or more years of direct service work
- OR current IRIS Consultant status
Skills
- Case management and care coordination
- Experience in disability or aging services
- Healthcare navigation and resource education
- Strong communication and cultural competence
- Bilingual abilities (e.g., Spanish, Hmong, ASL) are preferred
