About the role

We are seeking a compassionate and organized IRIS Consultant to support adults, older adults, and individuals with disabilities through Wisconsin's IRIS program.

Responsibilities

  • Build trusting relationships with participants
  • Develop and update individualized Support and Services Plans
  • Conduct regular contacts and in-person visits to promote self-determination and community inclusion
  • Monitor health, safety, and program outcomes
  • Educate participants on eligibility, resources, and program requirements
  • Collaborate with community organizations, healthcare providers, and support teams
  • Maintain accurate case documentation and ensure compliance
  • Assist with functional screens
  • Work independently within a team

Qualifications

  • Bachelor’s degree in Social Work, Human Services, or related fields with at least one year of relevant experience, or
  • Four or more years of direct service experience, or
  • Current IRIS Consultant certification in good standing
