About the role
We are seeking a compassionate and organized IRIS Consultant to support adults, older adults, and individuals with disabilities through Wisconsin's IRIS program.
Responsibilities
- Build trusting relationships with participants
- Develop and update individualized Support and Services Plans
- Conduct regular contacts and in-person visits to promote self-determination and community inclusion
- Monitor health, safety, and program outcomes
- Educate participants on eligibility, resources, and program requirements
- Collaborate with community organizations, healthcare providers, and support teams
- Maintain accurate case documentation and ensure compliance
- Assist with functional screens
- Work independently within a team
Qualifications
- Bachelor’s degree in Social Work, Human Services, or related fields with at least one year of relevant experience, or
- Four or more years of direct service experience, or
- Current IRIS Consultant certification in good standing
