About the role
Support adults, seniors, and individuals with disabilities through Wisconsin's IRIS program by developing and maintaining participant relationships, promoting self-determination and community inclusion, and ensuring health and safety.
Responsibilities
- Develop and maintain relationships with participants
- Assist in creating and updating individualized support plans
- Conduct regular contacts and visits
- Promote self-determination and community inclusion
- Monitor health and safety
- Educate participants about program eligibility and resources
- Collaborate with community partners
- Ensure accurate documentation
- Support annual assessments
Qualifications
- Bachelor's degree in a relevant field with at least one year of experience working with individuals with disabilities or older adults, or
- Four or more years of direct service experience, or
- Current IRIS Consultant status
Preferred skills include case management, care coordination, social work, healthcare navigation, and cultural competence. Bilingual candidates are encouraged to apply.
