About the role

Support adults, seniors, and individuals with disabilities through Wisconsin's IRIS program by developing and maintaining participant relationships, promoting self-determination and community inclusion, and ensuring health and safety.

Responsibilities

  • Develop and maintain relationships with participants
  • Assist in creating and updating individualized support plans
  • Conduct regular contacts and visits
  • Promote self-determination and community inclusion
  • Monitor health and safety
  • Educate participants about program eligibility and resources
  • Collaborate with community partners
  • Ensure accurate documentation
  • Support annual assessments

Qualifications

  • Bachelor's degree in a relevant field with at least one year of experience working with individuals with disabilities or older adults, or
  • Four or more years of direct service experience, or
  • Current IRIS Consultant status

Preferred skills include case management, care coordination, social work, healthcare navigation, and cultural competence. Bilingual candidates are encouraged to apply.
