About the role
We are seeking a compassionate and organized IRIS Consultant to support adults, seniors, and individuals with disabilities within Wisconsin's IRIS (Include, Respect, I Self-Direct) Program.
Responsibilities
- Build trusted relationships with participants and develop individualized support plans
- Assist with service coordination, self-determination, and community inclusion
- Conduct regular participant contacts, in-person visits, and monitor health and safety
- Educate participants on program requirements and available resources
- Collaborate with community organizations, healthcare providers, and support teams
- Maintain accurate documentation and ensure compliance standards
- Support annual functional screens and work independently within a team
Qualifications
- Bachelor’s degree in social sciences or related fields with at least one year of relevant experience or four or more years of direct service experience
- Experience in case management, care coordination, disability services, or long-term care is preferred
- Strong communication, relationship-building, and cultural awareness skills
- Proficiency with technology and a commitment to participant dignity and self-determination
- Bilingual candidates, especially in Spanish, Hmong, Arabic, Russian, ASL, or other languages, are encouraged to apply

- Ability to travel throughout the service area and meet auto insurance standards
