About the role
Full-time position supporting adults, older adults, and individuals with disabilities through Wisconsin's IRIS Program. Build trusted relationships, develop individualized support plans, and promote self-direction and independence while monitoring health and safety.
Responsibilities
- Build trusted relationships with participants
- Develop and update individualized support plans
- Conduct regular contact and in-person visits
- Promote self-direction and independence
- Monitor health and safety
- Educate participants about program resources
- Collaborate with community partners
- Maintain accurate documentation
- Assist with functional screens
Qualifications
- Bachelor's degree in Social Work or related field with at least one year of experience
- OR four or more years of direct service experience
- OR current IRIS Consultant in good standing
Preferred skills include case management, care coordination, disability services, and healthcare navigation. Strong communication, cultural awareness, and the ability to work independently are essential.
Additional skills
- Bilingual abilities (e.g., Spanish, Hmong, ASL)
- Proficiency with technology
- Valid driver's license with reliable transportation for travel
