IRIS Consultant - Hybrid, Full Benefits, PSLF Eligible

About the role

We are seeking a compassionate and organized IRIS Consultant to support adults, older adults, and individuals with disabilities through Wisconsin's IRIS Program.

Responsibilities

• Build trusted relationships with participants and develop individualized support plans
• Assist with program enrollment, updates, and educate participants about resources
• Monitor health, safety, and program outcomes
• Promote participant choice, independence, and community inclusion
• Collaborate with community organizations, healthcare providers, and support teams
• Maintain accurate case documentation and ensure compliance standards
• Conduct in-person visits and support functional screens as needed

Qualifications

• Bachelor’s degree in Social Work, Human Services, Psychology, or related fields with at least one year of experience working with individuals with disabilities or older adults, or four or more years of relevant experience
• Strong communication, relationship-building, and cultural awareness skills
• Proficiency with digital tools and a valid driver’s license for travel
• Preferred experience in case management, care coordination, or disability services
• Bilingual abilities in languages such as Spanish, Hmong, Arabic, Russian, or ASL are highly valued
