About the role
The IRIS Consultant supports adults, older adults, and individuals with disabilities through Wisconsin's IRIS program, focusing on case management, care coordination, social work, and community-based support to promote independence and self-direction.
Responsibilities
- Build trusted relationships with participants and develop personalized IRIS Support and Services Plans.
- Conduct monthly contacts and quarterly in-person visits to monitor health, safety, and program outcomes.
- Educate participants on eligibility, resources, and self-determination principles.
- Collaborate with community organizations, healthcare providers, and resource centers.
- Maintain accurate documentation and assist with functional screens as needed.
- Work independently within an interdisciplinary team to ensure participant well-being.
Qualifications
- Bachelor’s degree in Social Work, Human Services, Psychology, or related fields and experience, or equivalent experience in services for older adults or individuals with disabilities.
- Strong communication, relationship-building, and cultural awareness skills.
- Proficiency with technology and ability to manage a flexible schedule.
- Fluency in additional languages (e.g., Spanish, Hmong, ASL) is highly valued.
- Valid driver’s license, reliable transportation, and ability to travel within the service area.
