About the role
We are seeking a compassionate and organized IRIS Consultant to support adults, older adults, and individuals with disabilities through Wisconsin's IRIS (Include, Respect, I Self-Direct) Program.
Responsibilities
- Build trusted relationships with participants and develop individualized IRIS Support and Services Plans
- Conduct regular contact, including monthly check-ins and quarterly in-person visits
- Promote participant choice, self-direction, independence, and community inclusion
- Monitor health, safety, and program outcomes
- Educate participants on IRIS eligibility, resources, and enrollment requirements
- Collaborate with community partners, healthcare providers, and service agencies
- Maintain accurate documentation and ensure compliance with standards
- Support assessments such as Long-Term Care Functional Screens when needed
- Work independently within a team environment
Qualifications
- Bachelor’s degree in Social Work, Human Services, or related fields with at least one year of relevant supervised experience, or four or more years of direct service experience
- Experience in case management, care coordination, disability or long-term care, social work, or community services is preferred
- Strong communication, relationship-building, and cultural awareness skills
- Proficiency with digital tools and commitment to participant rights and self-determination
- Bilingual candidates in languages such as Spanish, Hmong, Arabic, Russian, or ASL are encouraged to apply

- Valid driver’s license and reliable transportation for travel within service areas
