About the role
This position provides skilled direct care services to individuals with developmental disabilities in an intermediate care facility (ICF) residential setting. Services include providing skilled supports to persons who have significant behavioral challenges and who have been determined to be dangerous to themselves or others, requiring close supervision. Customers include adults with intellectual and developmental disabilities (IDD) with a criminal history, members of their support teams, all Los Lunas Community Program (LLCP) staff, and other stakeholders.
Responsibilities
- Monitors medical, physical and emotional well being of individuals
- Implements individual program plans, behavioral support plans, and prescribed treatment and care plans
- Observes and reports changes in condition
- Assists individuals with activities of daily living
- Supports individuals in maintaining the highest level of function
