Intermediate Care Facility -Direct Care II (DOH/LLCP#15293)

About the role

This position provides skilled direct care services to individuals with developmental disabilities in an intermediate care facility (ICF) residential setting. Services include providing skilled supports to persons who have significant behavioral challenges and who have been determined to be dangerous to themselves or others, requiring close supervision.

Our customers include adults with intellectual and developmental disabilities (IDD) with a criminal history, members of their support teams, all Los Lunas Community Program (LLCP) staff, and other stakeholders.

Responsibilities

• Monitor individuals' medical, physical, and emotional well-being.
• Implement individual program plans, behavioral support plans, and prescribed treatment and care plans.
• Observe and report changes in condition.
• Assist individuals with activities of daily living.
• Support individuals in maintaining the highest level of function and stability within a structured environment.
• Provide accurate and timely documentation, including services performed, individuals' condition, behavioral observations, and progress toward goals in accordance with ICF regulatory, Medicaid, and agency requirements.
• Provide services in a 24/7 setting.

Requirements

• High School Diploma and one (1) year of experience as it pertains to the essential duties of the classification.
• Any combination of education from an accredited college or university in a related field and/or direct experience in this occupation totaling one (1) year may substitute for the required experience.
• Designated as safety sensitive; employment is subject to pre-employment and random drug/alcohol testing.
• Employment is subject to a criminal background check and is conditional pending results.
• Must possess and maintain a valid New Mexico Driver’s License.
• Must possess and maintain a current Defensive Driving Course Certificate from the State of New Mexico or obtain one as a condition of continued employment.
• Successful completion of Health Insurance Portability and Accountability Act (HIPAA) training is required.

Working Conditions

• Work is performed in an office setting as well as in the community-based homes of individuals.
• Ability to lift/push/transfer and/or carry up to 50 pounds without assistance and over 100 pounds with assistance.
• May be exposed to unpredictable violent behavior of individuals.
• Daily driving is required to meet the needs of individuals served; occasional overnight stay may be required.
• Specific geographic location for this position will vary depending on agency needs and utilization.
• Exposure to Visual/Video Display Terminal (VDT) and extensive personal computer and phone usage.
• Sitting, standing, walking, bending, and reaching are essential functions of this job.

Pay

$18.69 - $28.04 hourly / $38,885 - $58,327 annually.

This position is covered by a collective bargaining agreement, and all terms/conditions of that agreement apply.
