Intake Registered Nurse (RN)

Responsibilities

• Conduct comprehensive initial assessments of new clients, including medical history, functional status, and home environment.
• Develop, document, and update individualized care plans based on client needs and physician orders.
• Educate clients and families on care plans, medications, and home healthcare processes.
• Ensure all intake documentation is accurate, timely, and compliant with state and federal regulations.
• Cook up with physicians, care team members, and referral sources to facilitate seamless client admission.
• Assess eligibility for programs (Veterans, GAPP, CCSP, Medicaid/Medicare, private pay).
• Maintain confidentiality of patient records and comply with HIPAA guidelines.

Requirements

• A current and valid Registered Nurse (RN) license in the state of Vermont.
• A minimum of 1 year of clinical nursing experience (home healthcare or intake/admissions preferred).
• Strong assessment, documentation, and communication skills.
• The ability to work independently and collaboratively within an interdisciplinary team.
• Familiarity with state/federal home health regulations a plus.
• CPR Certification (current).
