Intake Coordinator - LPN

About the Role

The LPN Intake Coordinator is responsible for managing incoming referrals and orders, communicating patient and referral details to relevant teams, and acting as a liaison between clinical and marketing staff.

Responsibilities

• Obtaining and verifying demographic and clinical information from clients, physicians, and referral sources
• Researching coverage and pay sources, verifying policies
• Educating patients, families, physicians, and hospitals about home health services
• Ensuring accuracy of referral documentation and licensure verification

Qualifications

• Current, unrestricted LPN licensure
• At least 1 year of insurance or medical experience
• Valid driver’s license, vehicle insurance, and access to transportation

Preferred: CPR certification or ability to obtain within 90 days of hire.
