About the role
The LPN Intake Coordinator manages incoming referrals by collecting and verifying demographic, clinical, and insurance information from patients, physicians, and referral sources. They communicate effectively with clinical teams, educate patients and families about home health services, and serve as a liaison between clinical and marketing staff.

Requirements
- Unrestricted LPN license
- At least one year of insurance or medical experience
