Intake Coordinator - Clinical (Home Care), Full-Time

About the role

The Home Care Referral Coordinator manages patient referrals, ensuring accurate entry, scheduling, and communication among clinical staff, referral sources, and community partners.

Responsibilities

• Processes and documents new referrals promptly and coordinates care with patients and interdisciplinary teams.
• Interfaces with physician offices and community liaisons to assess, process, and facilitate referrals and admissions.
• Provides clear communication to patients, healthcare professionals, and community members regarding home care needs and benefits.
• Verifies insurance information, explains benefits, and maintains confidentiality per HIPAA regulations.
• Distributes referrals with complete background information and plans of care to appropriate clinicians.
• Works independently to meet deadlines, manages multiple tasks, and participates in clinical activities such as faxing, scanning, and emailing.
• Performs additional duties as assigned to support organizational needs.

Qualifications

• Associate’s/Technical Degree in nursing or related field required.
• Current RN, LVN, or LPN license in the practice state required.
• Preferred three years of experience in home care.
