Responsibilities
- Manages communication with physicians, patients, families and referral sources to ensure effective coordination of home care services referrals.
- System entry and transfer of pertinent information to client's physician, therapists and agency staff members.
- This position may have weekend responsibilities depending on the needs of the business.
- Respond to all electronic referrals in a timely manner.
- Answering phones and delivering 5-star customer service.
- Communication between Referral sources and locations to ensure proper documentation is obtained.
- Verification of Physicians
- Ensuring PCP to sign orders
- Last PCP appointment confirmation
- Confirming 90 days prior of confirmed 30 days post on community referrals.
- Verification of insurances to ensure coverage of services.
- Obtain Face to Face with all clients.
- Chart creation for patients to include Demographics and primary DX coding
Requirements
- High school diploma/GED or a college degree
- College degree preferred – Experience will be considered in placement of degree.
- Must be able to work after hours on-call schedule
- Proficient Microsoft Office skills
- Proficient typing skills

Compensation
- $24 an hour, plus bonus potential (up to 5% bonus based on performance)
- cell phone, on call and mileage reimbursement
- Up to 80 hours of vacation accrual in the first year, 48 hours of sick time accrual, 18-hour fun day and 18-hour inclusion day.
- Vacation hours accrual will increase to 120 hours of vacation after 1st year.
