About the Role
The Intake Coordinator plays a critical role in managing the initial client engagement process, ensuring a smooth and efficient intake experience. This position is responsible for gathering and verifying essential client information, coordinating with various internal departments, and facilitating timely communication to support service delivery. Minimum Qualifications
High school diploma or equivalent required; associate degree or higher preferred. Proven experience in a client-facing role, preferably in healthcare, social services, or related fields. Strong organizational skills with the ability to manage multiple tasks and deadlines. Excellent verbal and written communication skills. Proficiency with standard office software and client management databases.
Preferred Qualifications
Experience working with diverse populations and understanding of cultural competency. Familiarity with healthcare or social service intake processes and regulatory requirements. Certification in customer service or administrative support. Ability to speak a second language relevant to the client community. Experience with electronic health records (EHR) or case management software.
Responsibilities
Conduct initial client conversations to collect and verify personal, financial, and service-related information. Coordinate intake appointments and follow-up communications with clients and internal teams. Maintain accurate and confidential client records in compliance with organizational policies and legal requirements. Collaborate with service providers, and administrative staff to ensure seamless client onboarding. Identify and address potential barriers to service access, escalating issues as necessary to appropriate personnel. Prepare and submit intake documentation and reports to support program evaluation and funding requirements. Respond promptly to client inquiries and provide clear information regarding intake procedures and next steps.
