Key Responsibilities
- Manage and follow up on all assigned referrals through admission and start of care.
- Verify eligibility, authorizations, and payer requirements for home care services.
- Cookordinate communication between referral sources, patients, caregivers, managed care plans, and agency staff.
- Obtain and review all required documentation for admission.
- Ensure compliance with New York State Department of Health regulations and agency policies.
- Track referral status and maintain accurate documentation in agency systems.
- Answer incoming calls professionally and courteously, providing information regarding agency services.
- Work collaboratively with staffing, nursing, and administrative teams to ensure seamless service initiation.
- Maintain strong relationships with hospitals, rehabilitation facilities, MLTCs, physicians, and community referral sources.
- Meet intake and admission timelines while maintaining a high level of accuracy and customer service.
Qualifications
- Previous experience as an Intake Coordinator in a Licensed Home Care Services Agency (LCHSA), Managed Long-Term Care (MLTC), home care agency, or related healthcare setting.
- Strong understanding of the MLTC enrollment process and Medicaid eligibility requirements (Medicaid applications are not completed in-house).
- Knowledge of home care authorization processes, payer requirements, and referral management.
- Excellent follow-up skills with the ability to manage multiple referrals simultaneously.
- Strong verbal and written communication skills with a professional demeanor.
- Able to build and maintain relationships with referral sources and community partners.
- Detail-oriented with exceptional organizational and problem-solving skills.
- Proficiency with electronic health records (EHR) systems and Microsoft Office applications.
