Intact Case Manager

Intact Case Manager Job Description We are seeking a dedicated and experienced Intact Case Manager to join our team. The Intact Case Manager will be responsible for providing comprehensive case management services to families involved in the child welfare system. This includes conducting assessments, developing service plans, coordinating services, and monitoring progress towards goals. Responsibilities Conduct initial assessments to determine family needs and strengthsDevelop individualized service plans in collaboration with families and other service providersCoordinate and monitor services to ensure that families are receiving appropriate supportProvide crisis intervention and support to families as neededMaintain accurate and up-to-date case recordsParticipate in case conferences and team meetingsCollaborate with community partners to access resources and support for families Qualifications: Bachelor's degree in social work, psychology, or related fieldPrevious experience working with children and families in a social services settingKnowledge of child welfare laws and regulationsStrong communication and interpersonal skillsAbility to work independently and as part of a teamValid driver's license and reliable transportation We Invest in You!
