Responsibilities
- Maintain accurate, timely, and complete client records and data tracking related to performance outcomes.
- Communicate promptly and professionally with internal teams and external providers via phone, email, and in-person contact.
- Support clients with daily living skills such as meal planning, cooking, shopping, and household tasks, and teach skills where needed.
- Observe and report changes in client behavior, mood, and functioning. Maintain awareness of cultural and physical/mental health needs.
- Promote adherence to safety protocols and model appropriate safety practices.
- Provide individual and group counseling, case management, and skill development that fosters recovery, responsibility, and positive social engagement.
- Aid clients with system navigation (e.g., medical, psychiatric, legal, or detention facilities) and linkages to community support like peer-based groups (e.g., AA, NA, Wellness Centers).
- Facilitate groups, classes, or community-based activities as directed.
- Monitor and support medication adherence, including prompts, refills, proper storage, and reporting per program guidelines.
- Participate in program planning and represent the team on agency committees as assigned.
- Provide transportation using personal or agency vehicles and assist with the use of public transportation systems. Ensure clients attend to all medical and related appointments.
Qualifications
- Case Manager I (Training Track Eligible): Associate degree and no experience: Eligible with six-year training plan; Bachelor’s degree and no experience: Eligible with four-year training plan; Master’s degree and no experience: Eligible with two-year training plan.
- Individuals with partial experience in related fields may have their required training period reduced proportionally based on prior relevant experience.
- Training includes weekly case conferences, monthly supervision with APD or PD, all Buckelew and County-assigned trainings, and completion of Relias modules in areas such as trauma-informed care, crisis intervention, motivational interviewing, and cultural competence.
- Case Manager I (Fully Qualified MHRS): Must meet Medi-Cal criteria for a Mental Health Rehabilitation Specialist: Associate degree with six or more years of experience in mental health, physical restoration, social adjustment, or vocational adjustment; or Bachelor’s degree with four or more years of experience in those fields; or Master’s degree with two or more years of related experience.
- Case Manager II: Master’s degree or higher is required. Experience with individuals with mental health and/or substance use disorders is preferred. Licensure (MFT, LCSW, etc.) or progress toward licensure is preferred. Some program placements may require licensure based on contractual obligations.
