What the role will entail
- Perform initial and ongoing patient/family, physical, emotional, psycho-social, cultural and spiritual assessments.
- Ensure and coordinate the delivery of comprehensive, holistic services by the Interdisciplinary Hospice Team.
- Provide palliative nursing care and symptom control in accordance with physician orders and IDG plan of care.
- Document and enter into EMR all assessments and interventions before the end of the shift.
- Develop and prioritize the interdisciplinary care plan goals of the patient/family, hospice team and attending physician, prior to team meetings.
- Provide anticipatory guidance to the patient/family to assist in preparing for coping with events as they occur.
Who we are looking for
- Graduate of an accredited nursing school.
- Valid New York State nursing license.
- Minimum of at least 2 years of nursing experience required.
- Minimum of at least 1 year ICU, Med-Surg, ER or similar required; one year of hospice experience preferred.
- Venipuncture and IV placement certification preferred.
- Ability to exercise critical thinking skills.
- Skill to prioritize and handle multiple tasks effectively.
- Bilingual is a plus!
Work Environment/Physical Demands
- Work in fast paced environment.
- Ability to lift at least 30 pounds of supplies and/or equipment.
- Moderate/High level of possible exposure to infectious agents.
- Routine duties involve potential for direct exposure to blood or body fluids.
Hours
- Per Diem 7:00PM-7:30AM (12.5 hour shifts).
- 6 shifts per month.
- 2 shifts on the weekend.
- 1 holiday per year.
FLSA
- Non-Exempt
Pay
- $44.87-46/hr;
- 5% night differential for night shifts.
