About the role

Provides psychosocial assessments and social medicine services as part of the emergency department/CDU care team, addressing high-need patient populations.

Responsibilities

  • Participates in care planning, case finding, and referrals for patients with complex social needs.
  • Completes psychosocial assessments, develops and implements care plans, and documents interactions per standards.
  • Supports patients and families with coping strategies, social support, and community resource referrals.
  • Conducts consultations on abuse, neglect, advance care planning, and legal decision-making.
  • Assists with discharge planning, ensuring appropriate community placement and post-discharge support.
  • Contributes to quality improvement, performance audits, staff education, and departmental activities.

Requirements

  • Master’s degree in Social Work with a valid license (LCSW or equivalent).
  • Minimum one year healthcare experience.
  • Knowledge of medical terminology, psychosocial impacts of illness, community resources, and care processes.
  • Strong interdisciplinary collaboration, communication, and customer service skills.
