About the role
Provides psychosocial assessments and social medicine services as part of the emergency department/CDU care team.
Responsibilities
- Respond to referrals and conduct psychosocial assessments
- Develop and implement care plans
- Document patient interactions and interventions
- Assist patients with coping strategies
- Provide social consultations on issues such as abuse, neglect, and legal decisions
- Support discharge planning by evaluating patient needs and coordinating community resource referrals
- Ensure appropriate placements for patients
- Participate in performance improvement and quality initiatives
- Contribute to staff education and departmental activities
Requirements
- Master’s degree in Social Work
- Licensure as a Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW)
- Minimum one year of healthcare experience
- Basic Life Support (BLS) certification
