Inpatient/Social Services Social Worker - On-Call Variable Shift

About the role

Provides psychosocial assessments and social medicine services as part of the emergency department/CDU care team.

Responsibilities

• Respond to referrals and conduct psychosocial assessments
• Develop and implement care plans
• Document patient interactions and interventions
• Assist patients with coping strategies
• Provide social consultations on issues such as abuse, neglect, and legal decisions
• Support discharge planning by evaluating patient needs and coordinating community resource referrals
• Ensure appropriate placements for patients
• Participate in performance improvement and quality initiatives
• Contribute to staff education and departmental activities

Requirements

• Master’s degree in Social Work
• Licensure as a Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW)
• Minimum one year of healthcare experience
• Basic Life Support (BLS) certification
