Responsibilities
- Deliver comprehensive social work support by conducting psychosocial assessments, identifying patient needs, implementing appropriate interventions, follow-up care, and discharge planning.
- Collaborate with interdisciplinary teams to coordinate safe, timely care across the continuum, including post-acute services and community resources, while advocating for patient-centered care.
- Educate patients and families on healthcare decisions, available resources, and complex issues such as advanced directives, end-of-life care, and social concerns.
- Connect individuals to community services and agencies, ensuring continuity of care and smooth transitions.
- Support care coordination by managing length of stay, reducing readmissions, and ensuring appropriate levels of care within network systems.
- Maintain clear communication, thorough documentation, and compliance with regulatory and professional standards, while also contributing to staff education and professional development.
- Demonstrate knowledge of social work principles and methods and the ability to apply these in a health care setting.
Requirements
- Licensure or certification: Social Work Certification issued by the State of Wisconsin.
- Education: Master’s degree in social work from a Council on Social Work Education (CSWE) accredited school.
- Experience: 1 year of social work experience in healthcare.

Schedule
Part time, 20 hours per week, first shift, includes occasional Saturdays and holidays.
