Schedule

• Full-Time, permanent position (40 hours)
• Standard working hours: 8:00 AM – 4:30 PM EST, Monday–Friday, with rotating weekends (Saturday or Sunday)
• A weekday off is provided when working a weekend day so hours do not exceed 40 per week
• Onsite position at Sentara Norfolk General Hospital, 600 Gresham Drive, Norfolk, VA 23507

Responsibilities

• Responsible and accountable for the provision and facilitation of comprehensive care management, services and quality outcomes for patients across the healthcare continuum, with primary focus on patient transition to the appropriate level of care while facilitating clinical appropriateness and LOS
• Directly support hospital inpatients identified in need of comprehensive case management
• Promote effective utilization and monitoring of health services
• Collaborate and communicate with the healthcare team and patient/caregiver to manage care and transitions
• Develop and/or implement a comprehensive care plan based on assessment and evaluation to include physical, mental and psychosocial needs of the inpatient/caregiver

Qualifications

• Education: Bachelor's Degree in Nursing preferred
• Licensure: Registered Nurse (RN) in VA or NC or Compact/Multi-state REQUIRED
• BLS within 90 days of hire
• Case Management (ACM, CCM, CCCTM, CMAC, or CMGT-BC) certification required within one year of eligibility
• 1 year of nursing experience REQUIRED
• 1 year acute care case management preferred
