Registered Nurse - Case Manager Inpatient - RN

About the role

As a Registered Nurse - Case Manager Inpatient, you will be responsible for managing patient care plans, coordinating care among healthcare providers, and ensuring patients meet their health goals.

Responsibilities

• Develop and implement individualized care plans for patients
• Collaborate with interdisciplinary teams to ensure comprehensive patient care
• Monitor patient progress and adjust care plans as needed
• Communicate effectively with patients, families, and other healthcare professionals
• Ensure compliance with regulatory standards and policies

Requirements

• Bachelor’s degree in Nursing (BSN)
• Current RN license in Arizona
• At least 1 year of acute care nursing experience
• Experience in case management or related field preferred
