Overview

Sentara Norfolk General Hospital is hiring an Inpatient Case Manager/Registered Nurse (RN). This position is eligible for up to a $20,000 sign-on bonus for qualified candidates.

Schedule

Full-time, permanent position (40 hours). Standard working hours: 8:00 AM – 4:30 PM EST, Monday–Friday, with rotating weekends (Saturday or Sunday). A weekday off is provided when working a weekend day so hours do not exceed 40 per week.

Location

Onsite at Sentara Norfolk General Hospital, 600 Gresham Drive, Norfolk, VA 23507.

About the role

The Inpatient Case Manager RN is responsible and accountable for the provision and facilitation of comprehensive care management, services, and quality outcomes for patients across the healthcare continuum, with a primary focus on patient transition to the appropriate level of care while facilitating clinical appropriateness and length of stay. Directly supports those in hospital patients identified in need of comprehensive case management. Promotes effective utilization and monitoring of health services, collaborates and communicates with the healthcare team and patient/caregiver to manage care and transitions. Develops and/or implements a comprehensive care plan based on assessment and evaluation to include physical, mental, and psychosocial needs of the inpatient/caregiver. Functions in one of the following practice settings: Hospitals Only.

Responsibilities

  • Development and/or implementation of a comprehensive care plan based on assessment and evaluation, including physical, mental, and psychosocial needs of the inpatient/caregiver

Education

Bachelor's Degree in Nursing preferred.

Qualifications

  • Registered Nurse (RN) in VA or NC or Compact/Multi-state license required
  • BLS within 90 days of hire
  • Case Management certification (ACM, CCM, CCCTM, CMAC, or CMGT-BC) required within one year of eligibility

Experience

  • 1 year of nursing experience
  • 1 year acute care case management preferred
