Registered Nurse - Case Manager - Inpatient

Role Overview: The Registered Nurse (RN) Case Manager collaborates within an interdisciplinary team to coordinate patient care across the continuum, ensuring appropriate utilization management, resource allocation, and thorough clinical documentation. Key Responsibilities Facilitate patient care plans by assessing, planning, implementing, evaluating, and communicating progress.Lead care coordination efforts, advocate for patients and families, and promote best practices to achieve high-quality, cost-effective outcomes.Provide education on case and utilization management, participate in committees, research, and community activities.Bridge communication between providers, patients, families, and external agencies to ensure timely, high-quality care. Qualifications & Skills Graduate of an accredited nursing program or relevant RN experience if not accredited.One year of RN or LPN experience in an applicable setting; RN Case Management experience or fellowship completion preferred.Certification (CCM or ACM-RN) preferred; valid RN license required.Strong knowledge of nursing standards, ethical practice, and care coordination principles.Additional state licensure and certifications as needed. Additional Details: The role is full-time, exempt, with competitive salary, benefits, and scheduled shifts of four 10-hour days.
