Registered Nurse - Case Manager - Inpatient

Role Overview: The Registered Nurse (RN) Case Manager collaborates within an interdisciplinary team to coordinate patient care across the continuum, ensuring appropriate utilization management, care coordination, resource use, and clinical documentation. Key Responsibilities: Assess, plan, implement, evaluate, and communicate patient care plans based on professional standards.Lead and educate team members on case management principles, promoting best practices for quality, financial, and patient satisfaction outcomes.Facilitate communication among providers, patients, families, and external agencies to deliver high-quality, cost-effective care.Participate in committee work, research, and community involvement to advance practice standards.Ensure care delivery aligns with ethical and professional guidelines, including ANA and case management standards. Qualifications & Skills: Current RN license and relevant state licensure.Preferred: Baccalaureate nursing degree from an accredited program, CCM or ACM-RN certification, and 1-3 years of acute nursing or case management experience.Basic Life Support (BLS) competency and additional certifications as required.Demonstrated leadership, communication, and clinical judgment skills. Additional Details: Full-time position with dayshift hours, every fourth weekend, no call. Compensation ranges from $90,604.80 to $136,011.20.
