In-Home Nurse Practitioner or Physician Assistant (Part Time) - CT

About the role

Signify Health, part of CVS Health®, is seeking a Part-Time Clinician (Nurse Practitioner or Physician Assistant) to conduct In-Home Health Evaluations. You’ll engage with individuals in their homes, bridging gaps in care through flexible, comprehensive, and personalized health assessments. The role focuses on building trusted relationships to improve health outcomes without altering existing treatment regimens.

Responsibilities

• Conduct 45-minute in-home health evaluations, including medication and medical history reviews, physical evaluations, and diagnostic tests (e.g., diabetic eye exams, spirometry) as ordered by the individual’s health plan.
• Use an iPad with an intuitive clinical workflow for evaluations; no prescribing, lab ordering, or treatment plan alterations required.
• Travel locally and within licensed states to perform in-home visits; conduct virtual visits as needed.
• Provide Signify Health services, including Diagnostic & Preventive Services (e.g., spirometry, diabetic eye exams, bone density screening).
• Join a national network of clinicians dedicated to improving health outcomes and connecting individuals to appropriate care.

Requirements

• Active, unrestricted license(s) in coverage area(s).
• Board certification (required).
• Multi-state licenses (preferred).
• Able to cover the following primary location: Hartford, CT.

Pay

The typical pay range for this role is $46.03 - $99.14 per hour. The actual base salary offer will depend on factors including experience, education, geography, and other relevant considerations.

Schedule

• Part-time: 29.5 anticipated weekly hours.
