In-Home Health - Nurse Practitioner or Physician Assistant (Part Time)

About the Role

This position involves providing in-home health evaluations as a Part-Time Clinician (Nurse Practitioner or Physician Assistant). The clinician conducts 45-minute visits, reviewing medication and medical history, performing physical assessments, and administering diagnostic tests if required. The role emphasizes building trusted relationships to improve health outcomes and offers flexibility to travel locally and conduct virtual visits.

Responsibilities

• Deliver personalized health assessments in members' homes or virtually
• Utilize an iPad and clinical workflow for evaluations
• Provide diagnostic and preventive services such as spirometry and screenings
• Maintain compassionate engagement and adhere to role requirements
• Adapt to diverse home environments and safety considerations

Requirements

• Active, unrestricted license(s) in coverage area(s)
• Board certification (required)
• Multi-state licenses (preferred)
• Ability to cover specific primary locations
