In-Home Health - Nurse Practitioner or Physician Assistant (Part Time)

About the Role

A Part-Time Clinician (Nurse Practitioner or Physician Assistant) provides in-home health evaluations, engaging with patients in their homes to review medical history, conduct physical assessments, and perform diagnostic tests as needed. This role involves building trusted relationships, offering personalized health insights, and ensuring quality care delivery. Clinicians may conduct virtual visits and travel locally within licensed states.

Responsibilities

• Perform comprehensive health evaluations in patients' homes, lasting approximately 45 minutes
• Review medication and medical history, conduct physical assessments, and order diagnostic tests if required
• Utilize iPads and clinical workflows to document evaluations accurately
• Travel locally and conduct virtual visits as needed
• Provide diagnostic and preventive services, including screenings

Requirements

• Active, unrestricted licenses in relevant areas
• Board certification (required)
• Preferred multi-state licensing
• Ability to serve the Saint Cloud area
• Comfortable working in diverse home environments with pets, smoke, or allergens
