About the role
A Part Time Clinician (Nurse Practitioner or Physician Assistant) providing in-home health evaluations to help improve patient health outcomes.
Responsibilities
- Conduct 45-minute visits in patients' homes or virtually
- Review medical and medication history
- Perform physical evaluations
- Administer diagnostic tests as ordered
- Build trusted relationships, deliver personalized healthcare, and ensure patient comfort and safety in diverse home environments
Qualifications & Skills
- Active, unrestricted licensure
- Board certification
- Multi-state licenses preferred
- Ability to cover primary locations
- Strong communication skills
- Compassion
- Flexibility to travel locally and within licensed states
- Proficiency with clinical workflows and devices
Key Duties
- Engage with patients to assess health needs
- Provide diagnostic and preventive services
- Contribute to community health efforts
