In-Home Health - Physician (Part Time) - CT

About the role

Signify Health, part of CVS Health®, is seeking a Part-Time Physician to provide In-Home Health Evaluations. You’ll engage with individuals in the comfort of their own homes, helping bridge gaps in care by building trusted relationships and delivering flexible, comprehensive, and personalized health evaluations.

The visit lasts about 45 minutes and includes a medication and medical history review, a physical evaluation, and—if ordered by the person’s health plan—additional diagnostic tests such as diabetic eye exams or spirometry. You’ll use an iPad with an intuitive clinical workflow and are not required to prescribe medicine, order lab tests, or alter current treatment regimens.

As an in-home clinician, your primary workplace will be the private residences of health plan members. These environments may vary and could include household pets, secondhand smoke, or other airborne allergens. Candidates must be comfortable and safe performing duties in diverse home settings.

Responsibilities

• Conduct in-home health evaluations, including medication reviews, medical history assessments, and physical evaluations.
• Perform diagnostic and preventive services as indicated (e.g., spirometry, diabetic eye exams, bone density screenings).
• Travel locally and within licensed states to conduct visits, with flexibility for virtual visits as needed.
• Use an iPad and clinical workflow to document evaluations without altering patients’ current treatment plans.
• Build trusted relationships with patients to improve health outcomes and connect them to appropriate care.

Qualifications

• Active, unrestricted medical license (MD or DO) in the coverage area(s).
• Multi-state licenses preferred.
• Able to cover Hartford and the surrounding area.

Pay

The pay range for this role is $125.00 per In-Home Health Evaluation completed. Pay may vary based on experience, education, geography, and other factors. Additional reimbursement opportunities are available for completed Diagnostic and Preventive Services (DPS), as ordered.

This is a part-time position with anticipated weekly hours of 29.6.
