About the role

Provide skilled nursing care in home settings, focusing on adult patients with complex needs such as feeding tubes, tracheostomies, ventilators, and medication administration. Deliver and coordinate patient care in accordance with physician orders, assess patient conditions, document activities, develop and update care plans, and educate patients and families on disease management and self-care.

Responsibilities

• Deliver skilled nursing care to adult patients with complex medical needs in home settings.
• Coordinate and provide patient care in accordance with physician orders.
• Assess patient conditions and document all nursing activities.
• Develop, update, and implement individualized care plans.
• Educate patients and families on disease management, self-care, and health maintenance.
• Maintain compliance with all applicable regulations and standards.
• Practice infection control and ensure patient and personal safety.

Requirements

• Active, unrestricted Registered Nurse (RN) license.
• Current CPR certification.
• Minimum of six months of clinical experience; additional state-specific or discipline-specific experience preferred.
• Physical ability to lift up to 50 pounds, travel to patient homes, and function in diverse environments.
• Exposure to bodily fluids and infectious agents; adherence to health and safety protocols, including vaccination and testing requirements where applicable.
