Key Responsibilities

• Delivering and documenting nursing care according to care plans and physician orders.
• Assessing patient conditions, reporting changes, and updating care plans.
• Implementing appropriate nursing interventions to achieve clinical outcomes.
• Educating patients and families on disease management, self-care, and prevention.
• Ensuring infection control and maintaining confidentiality and professional standards.
• Participating in ongoing education and skill development.

Qualifications & Skills

• Active, unencumbered LPN/LVN license in the practicing state.
• Current CPR certification.
• TB test within the last 12 months.
• Reliable transportation and ability to travel.
• Preferred: Six months of recent clinical nursing or home health experience.
• Strong attention to detail, organization, communication, and problem-solving skills.

Physical & Environment Requirements

• Ability to lift 50 pounds, reposition patients, and move equipment.
• Proficiency in walking, standing, bending, and climbing stairs.
• Ability to respond effectively in emergency situations.
• Comfort working in diverse environments with possible exposure to bodily fluids and infectious diseases.
