About the role
We are seeking a Licensed Practical/Vocational Nurse (LPN/LVN) to provide skilled nursing care for pediatric patients in a home setting.
Responsibilities
- Deliver and document personalized patient care per physician orders
- Assess patient conditions, recognize changes, and report to supervisors or physicians
- Implement and update nursing care plans and take appropriate actions to ensure patient safety
- Educate patients and families on disease management, self-care, and prevention
- Maintain infection control standards and comply with policies, regulations, and the Nurse Practice Act
Requirements
- Current, unrestricted LPN/LVN license in the practicing state
- Current CPR certification and TB skin test within the last 12 months
- Preferred: six months of clinical experience, especially in pediatrics or home health
- Physical ability to lift 50 pounds, reposition patients, and perform necessary movements
- Reliable transportation and ability to travel

- Effective in diverse environments, capable of responding to emergencies
