About the role

Providing compassionate homecare to adult patients with complex needs such as feeding tubes, tracheostomy, ventilators, and medication administration. Ensuring clinical outcomes through direct patient care, documentation via electronic tablets, and adherence to care plans. Collaborating with a multidisciplinary team to deliver high‑quality nursing services.

Responsibilities

• Providing compassionate homecare to adult patients with complex needs such as feeding tubes, tracheostomy, ventilators, and medication administration.
• Ensuring clinical outcomes through direct patient care, documentation via electronic tablets, and adherence to care plans.
• Collaborating with a multidisciplinary team to deliver high‑quality nursing services.

Qualifications

• Active, unencumbered LPN/LVN license in the practicing state; transfer of compact licenses to your state within 90 days if applicable.
• Current CPR certification with hands‑on component; TB skin test within the last 12 months.
• Reliable transportation and preferred six months of hands‑on nursing experience.
